FISAP Form and Instructions
The Fiscal Operations Report and Application to Participate (FISAP) is used by schools to apply for Campus-Based Program funding for the upcoming award year and to report Campus-Based Program expenditures for the previous award year.
The FISAP Form and Instructions are provided on this page for informational purposes only. A school must complete and submit the FISAP via the Electronic Campus-Based (eCB) Web site at https://cbfisap.ed.gov.
