Subject: Reminder to Report Verification Results for Verification Groups V4 and V5 to FAA Access to CPS Online

In a November 6, 2014 Electronic Announcement we reminded institutions of their responsibility to report, using the FAA Access to CPS Online website (https://faaaccess.ed.gov), the verification results of identity and high school completion status for applicants who had been assigned to Verification Tracking Groups V4 or V5. This announcement reminds institutions that, if not already reported, they must report 2015-2016 V4/V5 verification results no later than September 24, 2016.

We also remind institutions that the V4/V5 reporting requirement continues for the current 2016-2017 FAFSA processing year and must be reported no later than 60 days following the institution's first request to the applicant to submit the required V4 or V5 identity and high school completion documentation. Institutions must update a previously reported V4 or V5 status for any applicant whose status changes within 30 days of the change.

Information about V4/V5 reporting for the upcoming 2017-2018 FAFSA processing year will be forthcoming.

Institutions must report the verification results of identity and high school completion status, as described below, for any applicant for whom the institution received an ISIR with a Verification Tracking Group of V4 or V5, and for whom the institution requested verification documentation. Institutions should only use this reporting function for applicants where the CPS placed the applicant in Verification Tracking Groups V4 or V5. It should not be used in instances where the institution selected the applicant for verification of identify or high school completion status.

As we had stated in earlier communications, verification of an applicant’s high school completion status is not required for subsequent years if the institution successfully verified and documented the applicant’s high school completion status for a prior award year. However, the institution must report the verification results for the subsequent year as if verification was satisfactorily completed for that year.

Note that an applicant’s identity must be verified and the verification results reported each year even if it had been verified in the past.

This V4/V5 reporting is in addition to required verification results reporting to the Common Origination and Disbursement (COD) System.

Conditions for Each Identity Verification Results Value

The following describes the circumstances that apply for each of the V4 and V5 verification results values that institutions must use when reporting using FAA Access to CPS Online.

Value = 2 Verification of both identity and high school completion status were satisfactorily completed remotely by use of a notary, and no issues were found. The applicant submitted an original notarized Statement of Educational Purpose, a copy of his/her government-issued photo identification, and acceptable documentation of high school completion status as indicated on the ISIR.
Value = 3 Verification was attempted, and no issues were found with high school completion, but issues were found with identity. The applicant presented (not necessarily in person) acceptable documentation of high school completion status as indicated on the ISIR. No documentation of identity was submitted or the documentation that was presented was not acceptable and no signed or notarized Statement of Educational Purpose was received.

NOTE: If issues were found with both identity and high school completion status, use Results Value 3.
Value = 4 Verification was attempted, and no issues were found with identity, but issues were found with high school completion status. The applicant presented (either in person or with an original notarized statement) a signed Statement of Educational Purpose with an acceptable government-issued photo identification. Either no documentation of high school completion status was presented or the documentation that was presented was not acceptable.
Value = 5 The applicant did not respond to the request for documentation or the applicant could not be located. Verification of neither identity nor high school completion status was completed and no signed or notarized Statement of Educational Purpose was received because the applicant did not respond to the request for documentation or the applicant could not be located.
